FUE vs. DHI: which hair transplant method is right for me?

Anyone deciding on a hair transplant faces an important question: which method is right for me? The two most common techniques are FUE (Follicular Unit Extraction) and DHI (Direct Hair Implantation). Both have their advantages and disadvantages, and the decision depends on individual factors such as hair type, the pattern of hair loss, budget and personal preference. In this article we compare FUE and DHI in detail so that you can choose the method that is best for you.

FUE (Follicular Unit Extraction): the proven standard method

FUE is the most widely used hair transplant method. Individual hair follicles are taken from the back of the head with a micropunch (0.7–1.0 mm), prepared under the microscope and then placed in small channels in the recipient area. About 70–80% of all hair transplants worldwide are carried out with FUE.

How the FUE method works:

  1. Extraction:
    • Individual hair follicles are taken from the donor area with a special micromotor (0.7–1.0 mm in diameter).
    • The extraction is carried out under local anaesthetic and is painless.
    • Only tiny, barely visible scars remain.
  2. Preparation:
    • The extracted follicles are prepared under the microscope and sorted by size and quality.
    • This step takes about 1–2 hours.
  3. Implantation:
    • Small channels (incisions) are created in the recipient area with a fine needle or blade.
    • The follicles are carefully placed into these channels. This requires a great deal of precision and experience

Advantages of FUE

  • A proven method: in use for more than 20 years
  • Barely visible scars: only tiny, dot-shaped scars in the donor area
  • Natural results: carried out correctly, indistinguishable from natural hair
  • Flexibility: suitable for all hair types and large procedures
  • Cheaper than DHI: because it takes less time

DHI (Direct Hair Implantation): the more precise alternative

DHI stands for “Direct Hair Implantation” and is a further development of the FUE method. The decisive difference lies in how the hair is placed: with DHI the follicles are implanted directly into the scalp with a special implantation tool (Choi implanter), without channels having to be created first.

How the DHI method works

  1. Extraction:
    • As with FUE, individual hair follicles are taken from the donor area.
  2. Preparation:
    • The follicles are prepared and sorted under the microscope.
  3. Implantation:
    • With the Choi implanter (a special pen-like tool) the follicles are implanted directly into the scalp.
    • The implanter creates the channel and places the follicle in one step.
    • This allows more precise placement and reduces the time the follicles spend outside the body.

Advantages of DHI

  • Greater precision: the implanter allows the follicles to be placed more exactly
  • Faster healing: as no separate channels are created, the scalp heals more quickly
  • Lower risk of losing follicles: the follicles spend less time outside the body
  • Denser results: more precise placement allows more follicles per cm² to be implanted
  • No shaving required: with women or with small procedures, shaving can be avoided

FUE and DHI compared directly

  • Cost: FUE €1.00–2.00/graft, DHI €1.50–2.50/graft in Croatia
  • Length of surgery: FUE 6–8 h, DHI 8–10 h
  • Healing: FUE 10–14 days, DHI 7–10 days
  • Shaving: FUE: yes (donor and recipient area), DHI: often only the donor area (with women sometimes without shaving)
  • Large procedures: FUE is more efficient from 3,000+ grafts

Who is each method suited to?

FUE is ideal for:

  • Patients with extensive hair loss: with 3,000–4,000+ grafts, FUE is more efficient
  • Price-conscious patients: FUE is cheaper than DHI
  • Men with typical male hair loss: a proven method for most cases
  • Patients with thicker hair: the follicles are more robust and less prone to damage

DHI is ideal for:

  • Patients who want maximum density: more precise placement allows more follicles per cm² to be implanted
  • Women: shaving can often be avoided, which is more discreet
  • Patients with fine hair: more precise implantation reduces the risk of losing follicles
  • Smaller procedures: with 1,500–2,500 grafts, DHI is more efficient
  • Patients who want to be presentable quickly: faster healing thanks to the minimally invasive technique

Price comparison: Croatia vs. German-speaking countries

In Croatia and Slovenia both methods are 40–60% cheaper than in Germany or Austria. FUE costs €2,000–5,000 there for 2,000–3,000 grafts. GO Medics helps you find the right clinic — request a free consultation.
