Am I a candidate for an eyelid lift?

Good candidates are healthy adults with excess eyelid skin, protruding fat pads (eye bags) or age-related sagging around the eyes, who have realistic expectations. It is not suitable for patients with: Severe, untreated dry eyes Uncontrolled glaucoma or other serious eye conditions Active retinal problems Poorly controlled diabetes or thyroid conditions (thyroid eye disease) Unrealistic expectations Active smokers (must stop before surgery)

Which technique do you recommend in my case and why?

The choice of technique depends on individual anatomy and goals: Upper eyelid lift: removal of excess skin, if needed fat and muscle; used for hooded lids, drooping upper eyelids and a restricted field of vision Lower eyelid lift: removal/redistribution of fat, skin removal if needed; used for eye bags, wrinkles and sagging under the eyes Transconjunctival: incision inside the lower lid, no visible scar, fat removal only, no skin removal needed Transcutaneous: incision just below the lashes, skin removal possible, removes excess skin on the lower lid Combined surgery: both lids lifted at the same time, comprehensive rejuvenation of the whole eye area

How long does the operation take? What anaesthetic is used?

The procedure typically takes 45 minutes to 2 hours, depending on the extent (upper lids only, lower lids only or both). The operation is usually performed under local anaesthetic (with or without sedation/twilight sleep) — the patient stays awake but feels no pain. For extensive procedures, or on request, a general anaesthetic is also possible.

How long does recovery take? When can I work again?

Most patients need 10–14 days for the first stage of recovery, feel well enough for light activities or office work after 7–10 days, and can use light make-up and do light sport after 2 weeks. After 4–6 weeks most activities are possible again, but the final results only show after 3–6 months, once all the swelling has gone.

Are the scars visible? Where do the incisions run?

When performed correctly the scars are barely visible — on the upper lid the incision runs in the natural lid crease, so the scar is practically invisible when the eye is open. On the lower lid the incision is made just below the lash line (1–2 mm) or inside the lid (transconjunctival, no visible scar).

How long do the results last?

The results are long-lasting — upper lids usually stay tight for 5–7 years, often 10–15 years or longer, lower lids frequently for a lifetime, because the fat tissue removed does not grow back. The natural ageing process continues, however, so the skin can slacken somewhat again over the years.

What risks and complications are there?

As with any operation there are risks, but with experienced surgeons blepharoplasty is safe and damages neither the eyes nor eyesight.

Is the operation covered by health insurance?

An aesthetic eyelid lift (for cosmetic reasons) is usually not covered by statutory health insurance and has to be paid for privately. With a medical indication (severely drooping upper lids that restrict the field of vision), the upper eyelid lift can be covered by statutory insurance — this requires an ophthalmologist's certificate about the restricted field of vision and prior approval from the insurer.

What does the operation cost?

Costs vary widely depending on the country, the clinic, the surgeon and the extent of the surgery: in Austria and Germany the cost per eye is from €1,500 up to about €3,000 (upper lid), from €2,000 up to about €4,000 (lower lid) and from €3,000 up to about €6,000 (combined). In Croatia, by contrast, the cost is only from €1,000 up to about €2,400 (upper lid), from €1,500 up to about €3,000 (lower lid) and from €2,400 up to €4,500 (both eyes) — on average about 35% below the prices in Germany or Austria.
